Supporting Financial Documentation
In addition to your Sworn Financial statement, you will need to provide a copy of the following documents to your spouse, where applicable.
You must provide a copy even if your spouse already has access to the document.

- Recent pay stubs, showing gross pay and all withholdings
- Recent statement from other income sources, including unemployment insurance, disability, TANF, or worker’s comp, if applicable.
- Most recent bank statement for all your bank accounts, including checking, savings, and business accounts.
- Last three income tax returns and corresponding W2s
- Most recent statement for all your outstanding debts, including mortgage, credit cards, school loans, medical bills, auto loans, and any in collections.
- Documentation for any investments, including retirement accounts, stocks, bonds, CDs, or other investments.
- Documentation of insurance policies
- Documentation of expenses for child care, if applicable
- Documentation of expenses for education, if applicable
